Addressing Healthcare’s Blindside in Albuquerque’s South Side

Addressing Health Care’s Blindside addressed chronic disease and self-management through a multi-sector collaboration by establishing a referral tracking system and a Mobile Farmers Market. The Wellness Referral Center (WRC), an integrated chronic disease management referral system, was designed and implemented to link clinics to community resources in order to improve health outcomes. Over 1500 referrals from 15 referring entities have been processed since 2016. The Mobile Farmer’s Market improved healthy food access by distributing food grown by local farmers in target neighborhoods. SNAP, WIC, and other assistance methods were accepted. Last year, 1,000 unique visitors bought produce from the market.

Impact

The project has processed over 1,400 referrals to 20 community resources including chronic disease management classes, healthy cooking classes, physical activity opportunities, and the Mobile Farmer’s Market. Over 1,700 people have gained access to healthy food grown by local farmers. Ultimately, there is now a cohesive collaboration of partners poised to improve health and wellness in Albuquerque’s south side.
